The Healthy Women Healthy Families (HWHF) Initiative works toward improving maternal and infant health outcomes for women of childbearing age and their families, while reducing racial, ethnic and economic disparities in those outcomes through a collaborative coordinated community driven approach through the use of Community Health Workers and Central Intake Hubs.
We focus on women of childbearing age and their families, addressing their unique needs throughout preconception, pregnancy, and postpartum stages.
NDFSC’s Community Health Worker connects women and families with personalized care and support, offering reproductive health information, individual support plans, and workshops. We ensure they access local resources and provide breastfeeding and centering support.
Our goal is to create a supportive community for women before, during, and after pregnancy to reduce maternal and infant deaths. We foster connections through events like Community Baby Showers, Holiday Giveaways, and Mommy & Me playdates.
Outreach, screening/referral, and case management using a strength-based approach to support maternal well-being and promote healthy infants. Collaboration with the Partnership for Maternal Child Health, with funding support from The Henry and Marilyn Taub Foundation.
